How should modifier 59 be reported under the National Correct Coding Initiative (NCCI) program?

Modifier 59 is used to indicate a "distinct procedural service". Modifier 59 may not be appended to the evaluation and management (E&M) services Current Procedural Terminology (CPT) codes 99201-99499 or radiation treatment management CPT code 77427.
