Who do I contact if I have questions about supporting documents required to be submitted with my revalidation application?

Questions related to supporting documents, application fees and the status of your revalidation application should be directed to your Medicare Contractor.  Medicare Contractor contact information for each State can be found at http://www.cms.gov/Medicare/Provider-Enrollment-and-Certification/MedicareProviderSupEnroll/downloads/contact_list.pdf on the CMS website.


The External User Service (EUS) is able to assist with questions about logging into PECOS and the Authorized Official process in the PECOS Identification and Authentication (I&A) application.
