FAQ

If I inappropriately/erroneously obtained an NPI and later found that I am not eligible for an NPI, how can I deactivate the NPI?

If an NPI were obtained and the applicant found that he/she/it should not have obtained one, the NPI holder should send a request to the NPI Enumerator to have the NPI deactivated. The NPI holder can do this at any time by completing the NPI Application/Update Form (CMS-10114) for deactivation (see Section 1A3 and application instructions for details regarding the completion of the application for deactivation purposes). The application form can be downloaded at http://www.cms.gov/Medicare/CMS-Forms/CMS-Forms/downloads/CMS10114.pdf, or a copy can be requested from the NPI Enumerator by contacting them at 1-800-465-3203.
